A manuscript needs an architecture, not another checklist

When the review is finished, the writing problem changes. The task is no longer to decide which studies are eligible, how risk of bias should be assessed, or which synthesis model should be used. The task is to turn decisions and outputs that already exist into one scientific report in which the rationale, methods, results, displays, limitations, conclusions, supplements and declarations agree with one another.

PRISMA 2020 provides the principal reporting framework for this work, but it does not prescribe one universal journal layout. Its checklist organizes reporting recommendations under familiar sections such as Introduction, Methods, Results and Discussion, while journals remain free to use different heading structures and submission formats.12 The current Cochrane Handbook makes the same practical distinction visible from another direction: completed-review Methods should describe what was actually done, Results should present all relevant findings in a logical way, and Discussion should interpret the findings while considering both limitations of the included evidence and limitations of the review process.3

That means a useful manuscript template cannot simply reproduce the 27 PRISMA items. Doing so would duplicate a reporting checklist rather than solve the drafting problem. It also cannot hard-code one journal’s headings, word limits, figure caps or AI policy, because those requirements vary and can change. The practical solution is to separate a stable scientific core from a configurable journal layer.

What this resource does

This template organizes the manuscript as a working system: a configurable journal shell, section-specific drafting prompts, an explicit Methods–Results correspondence matrix, an evidence-display inventory, a supplement map, a declaration module and a final consistency audit. It does not calculate PRISMA adherence, assess methodological quality, perform statistical analyses or guarantee acceptance.

Three rules before you begin drafting

Write the review you actually completed

Final Methods should describe what was done, not simply preserve protocol language written prospectively. Material deviations or unperformed planned analyses should be made visible rather than silently disappearing.3

Make every major method meet its result

Study selection, appraisal, synthesis, heterogeneity investigations, sensitivity analyses, reporting-bias assessments and certainty assessments should have corresponding Results when they were conducted, or an explanation when a planned procedure was not performed.

Use displays to carry detail

Results prose should orient the reader to the important findings rather than reproduce every value already available in tables or figures. Detailed search strategies, excluded-study lists and extended technical material can be mapped to supplements where appropriate.3

Methodological boundary

A coherent manuscript can still describe a poorly conducted review. This template improves organization, traceability and reporting discipline; it does not repair inappropriate eligibility criteria, flawed appraisal, incorrect statistical models or unsupported certainty judgments. Those problems require the relevant methodological standards and analyses.

1. Define the target-journal shell

Journal constraints are not methodological constants. Record the rules that apply to the actual article type you intend to submit rather than relying on remembered requirements from another journal.

Journal rules change. The workspace records constraints rather than hard-coding them. Recheck the journal instructions immediately before submission.
2. Establish manuscript identity and front matter
Writing job: identify the work precisely. PRISMA 2020 Item 1 requires the title to identify the report as a systematic review. Mentioning a meta-analysis is informative when relevant, but PRISMA 2020 does not require every systematic-review title to contain that term.1
Detailed abstract drafting is intentionally outside this template’s scope.
3. Draft the Introduction as a rationale-to-objective argument
Writing job: move the reader from the problem to the unresolved evidence question. The Introduction should justify the synthesis, not become another exhaustive literature review.
4. Draft Methods as the record of what was actually done

The current Cochrane reporting guidance recommends writing completed-review Methods in the past tense and describing the procedures that produced the present results. The prompts below organize reporting; they do not teach how to perform those methods.3

5. Draft Results around the evidence actually produced
Writing job: tell the reader what the review found. Organize the section logically by the review question, comparison, population, outcome or synthesis structure. Report findings regardless of whether they are statistically significant, large or favorable.3
6. Methods–Results correspondence matrix

This matrix does a different job from a PRISMA checklist. It asks whether the manuscript’s own Methods and Results agree. A method can be fully reported yet still lack its expected result; a result can appear without a sufficiently described method; a planned analysis may legitimately be absent if the reason is stated.

7. Coordinate tables and figures with the manuscript
Writing job: assign each display a purpose. Detailed evidence belongs in the display; prose should tell the reader why the display matters and which patterns deserve attention.

8. Draft Discussion and conclusion without turning them into a second Results section
Spin can arise through misleading reporting, interpretation or extrapolation; systematic-review conclusions are particularly vulnerable when they imply benefits or safety beyond what the evidence supports.56
9. Build the supplementary-material map

The map below is a MetaSyn operational device, not a PRISMA-prescribed table. Its purpose is to prevent supplements from becoming detached files whose relationship to the manuscript is unclear.

PRISMA 2020 separately addresses support (Item 25), competing interests (Item 26), and availability of data, code and other materials (Item 27). Journals may add their own declaration formats and requirements.1

11. Run the final manuscript consistency audit

This is not a second PRISMA audit. It is a manuscript-level verification step. Cochrane’s current reporting guidance explicitly emphasizes consistency of key results, outcomes and conclusions across the review’s reporting surfaces.3 Abstract/full-text research also shows that Results and Conclusions are frequent locations of inconsistency.8

How to use the template without letting the template write the review

Start with the journal shell only after the completed review has a stable set of analyses and evidence displays. The journal fields tell you what the submission environment permits, but they should not determine the scientific argument. If a journal allows only four figures, for example, that may change which display moves to a supplement; it should not change which results the review reports.

Next, draft the Introduction from the review question backward. The objective should be visible as the destination of the rationale. This is different from starting with several pages of background and trying to discover the purpose at the end. A systematic review exists because a defined evidence problem required synthesis. The Introduction should make that problem intelligible without repeating the evidence synthesis before the Results section has begun.

The Methods and Results sections then become the structural spine. Current Cochrane guidance is especially useful here because it treats the completed Methods as an account of what actually produced the review’s findings and explicitly requires authors to explain differences between the protocol and completed review.3 The correspondence matrix in this resource operationalizes that idea at manuscript level. It does not judge whether a subgroup analysis was statistically appropriate; it asks whether a subgroup result that appears in the Results has a transparent methodological origin.

The same principle prevents a different failure: planned analyses vanishing without explanation. A review may have prespecified meta-regression and then include too few studies to make it defensible. That is not automatically a methodological defect. The writing defect occurs when the Methods continue to promise the analysis or when the analysis quietly disappears and the reader is left to reconstruct what happened. The manuscript should report the final analytical reality.

Evidence displays should then be treated as part of the argument rather than as appendages. A study-characteristics table allows the prose to summarize meaningful variation instead of describing each study individually. A forest plot can carry study-level estimates while the Results text states the main effect, amount of evidence, uncertainty and aspects of the finding that matter for interpretation. A supplement can carry complete search strategies or extended technical detail while the main manuscript tells readers exactly where those materials are located.

The Discussion solves a different problem. Its job is not to restate all findings in more elegant language. It should identify the results that answer the review question, interpret them in context, explain how limitations constrain their meaning and distinguish problems in the primary evidence from limitations introduced by the review process. The current JBI Manual makes the same distinction, instructing authors to discuss both limitations of included studies and limitations arising from the review process itself.7

Finally, treat declarations as part of scientific transparency rather than administrative material added after the paper is written. PRISMA requires reporting support, competing interests and availability of review materials in separate items.1 ICMJE adds current editorial guidance concerning authorship and AI-assisted technologies. In its current recommendations, authors should disclose AI use at submission, describe how it was used in the submitted work where applicable, report writing assistance in acknowledgments, and describe AI used for data collection, analysis or figure generation in Methods.9 Those are ICMJE recommendations, not new PRISMA checklist items, and target-journal policies still need to be checked.

The template is finished when the manuscript, not the form, is coherent

Do not aim to fill every box. Remove or adapt prompts that genuinely do not apply to the review type. The completed workspace is useful only if it exposes the structure of the actual paper and makes gaps easier to detect before submission.

Frequently asked questions

Does PRISMA 2020 require a systematic review manuscript to use IMRaD headings?

No. PRISMA 2020 organizes reporting recommendations under familiar headings, but its purpose is to specify what should be reported rather than impose one universal journal format. Use the target journal’s required headings while ensuring that the applicable PRISMA information remains reported and findable.12

Should the title always say “systematic review and meta-analysis”?

PRISMA 2020 Item 1 requires the title to identify the report as a systematic review. The expanded guidance suggests that authors may add information about the analysis method, such as indicating that meta-analysis was performed, when this improves identification. A systematic review and a meta-analysis are not interchangeable terms.

Why is the Methods–Results matrix different from the PRISMA tracker?

The PRISMA tracker asks whether reporting items are present and where they are reported. The correspondence matrix asks whether the manuscript is internally coherent: does each important method have a corresponding result or a documented reason for absence, and does each important result have a transparent methodological origin?

Should Methods reproduce the protocol word for word?

No. The completed manuscript should describe what was actually done. The protocol remains important as the prospective record, but amendments and planned procedures that could not be implemented should be reported transparently rather than hidden by copying prospective wording into the final Methods.3

Should every result in a table or forest plot also be repeated in prose?

Usually not. Prose should help readers identify the important patterns and findings, while tables and figures can carry detailed structured information. Current Cochrane guidance specifically cautions against simply repeating all synthesis results in the Discussion and encourages concise narrative summaries in Results.3

Does every systematic review need a GRADE section?

No. Certainty or confidence assessment depends on review type, methodology and the framework used. If the review used GRADE or another formal confidence system, report the methods and results consistently. Do not add a framework retrospectively merely because a manuscript template contains a prompt.

Does PRISMA require authors to make data and code publicly available?

PRISMA Item 27 requires authors to report which review materials are publicly available and where they can be found. It does not create a universal requirement that every dataset or code file must be publicly released. Journal, funder or institutional policies may impose additional sharing requirements.1

Where should AI use be disclosed?

Follow the target journal’s current policy. Under current ICMJE guidance, authors should disclose AI use at submission and describe how it was used in the submitted work. Writing assistance is described in acknowledgments, while AI used for data collection, analysis or figure generation is described in Methods. AI systems cannot be listed as authors.9

Does completing this template make a manuscript publication-ready?

No template can certify publication readiness. This resource helps organize and cross-check the report. The underlying review methods must still be appropriate, the PRISMA checklist must be audited separately, journal requirements must be checked, and all factual and numerical content must undergo final human verification.

The manuscript is complete when its parts stop contradicting one another

A well-structured systematic-review manuscript does more than contain the expected sections. The objective matches the review that was performed. Methods explain how each major result was produced. Results point readers to the right evidence displays without duplicating them. Discussion interprets rather than re-reports. Limitations explain how uncertainty changes meaning. Conclusions stay within the evidential boundaries of the review. Supplements, registration records, availability statements and declarations remain connected to the manuscript rather than existing as detached administrative files.

That is the standard this template is designed to support: not a uniform manuscript, but a coherent one.
